This trial aims to understand the role of the NLRP3 inflammasome in CIM and whether physical therapy can help prevent or reduce muscle loss by affecting related gene activity. The study compares two types of physical therapy for critically ill patients at risk of CIM during early ICU stay. One group receives standard physiotherapy, while the other group gets enhanced therapy including twice-daily 60-minute sessions of servo-assisted cycling using the Motomed Letto device for 7 consecutive days. Additionally, a control group of patients without CIM will be included for comparison. Muscle biopsies and blood samples will be collected to analyze molecular markers related to muscle atrophy and inflammasome activity. Participants will undergo clinical evaluations using muscle ultrasound and the SOFA scale to diagnose CIM. Muscle strength and physical function will be tracked over time, alongside molecular and gene expression analyses from tissue samples taken at the start and end of the intervention.
