Purpose of the Role:
You will provide psychosocial support and counselling under the purview of Psychosocial Services (PSS), which includes providing psychosocial services to patients and families under our hospice service, and beneficiaries of our psychosocial services. You are also expected to provide support in major organisation-wide events like fundraising and public forums, and in other projects as assigned by the Manager.
Key Responsibilities:
1. Psychosocial Care & Counselling
- Able to develop professional and engaged relationships with clients and other key service stakeholders.
- Provide and facilitate psychosocial assessments to assess cancer patient’s impact of illness and progression.
- Conduct on-going individual and/or family counselling through face-to-face sessions and home visitations to help navigate the bio-psycho-social-spiritual impact of illness.
- Conduct risk assessment for complex cases by considering integration of a range of information independently.
- Work closely with multi-disciplinary team and/ or agencies to deliver care/ case plans.
- Lead case conferences as part of a multi-disciplinary team.
2. Community Work: Remembrance
- To work with internal team & cross team/s for planning and execution of remembrance segment for Relay for Life (RFL) annually
3. Research
- To support research projects undertaken by PSS Hospice team or other initiatives This includes, but is not limited to recruitment, development and coordination of research project within internal and cross-team, data analysis, research writing and/or review.
4. Other Roles
- To identify and encourage suitable clients and/or caregivers to be part of relating to their lived-in experience as patients/caregivers to spread awareness on cancer through multiple platforms.
- To be involved in palliative related events and initiatives, such as World Hospice and Palliative Care Day organised by Singapore Hospice Council. Involvement may include events-related booth activities
