Purpose of the Role:

Reporting to the Senior Manager of Psychosocial Services, you will provide psychosocial care and counselling under the purview of Psychosocial Services, which includes providing psychosocial services to patients and families under our SCS cancer rehabilitation service. Cases may also come from external referrals (such as hospitals, community partners, etc) or other internal referrals (e.g. Welfare Team, emails, SCS hotline, etc).

You will be involved in key programmes that supports cancer survivorship.

You are also expected to provide support in major organization-wide events like fundraising and public forums, and in other projects as assigned by the Manager.

Key Liaison

You are part of the Cancer Survivorship Team, working closely with relevant internal and external stakeholders:
- Clients and their families
- Cancer Rehabilitation team and internal team within psychosocial services
- Government agencies and hospitals
- Other social service agencies or community partners

- Psychosocial Care & Counselling
- Able to develop professional and engaged relationships with clients and other key service stakeholders.
- Provide and facilitate psychosocial assessments to assess cancer patient’s impact of illness and progression.
- Conduct on-going individual and, or family counselling through face-to-face sessions and home visitations to help navigate the bio-psycho-social-spiritual impact of illness.
- Conduct risk assessment for complex cases by considering integration of a range of information independently.
- Work closely with multi-disciplinary teams and, or agencies to deliver care or case plans.
- Lead case conferences as part of a multi-disciplinary team.
- Prepare social reports and ensure documentation is in place.
