The Role
To provide a safe and effective patient focused specialist clinical pharmacy service to a specified patient group(s) supporting medicines governance and quality improvement activities within specialist area(s) to ensure safe, effective and patient-centred systems in place
In This Key Role, You Will
- To deliver patient-centred specialist pharmaceutical care to patients in specified clinical area(s).
- To communicate effectively, placing the person at the centre of any interaction.
- To deliver patient education to ensure the safe and effective use of medications and devices.
- To collaborate with the wider pharmacy and multidisciplinary team; actively participating in consultant ward rounds and multidisciplinary team meetings.
What You'll Bring
- Masters of Pharmacy Degree
- Registered member of General Pharmaceutical Council
- Independent prescribing qualification or must work towards
- Evidence of postgraduate training and professional development in clinical pharmacy practice e.g. Post Registration Foundation Framework/ 1st year clinical diploma
- Relevant post registration experience in clinical pharmacy.
- Ability to effectively monitor patients' therapeutic regimens in order to ensure effectiveness and reduce toxicity
- Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment.
