The Role
To provide a safe and effective patient-focused specialist clinical pharmacy service to a specified patient group(s), supporting medicines governance and quality improvement activities within specialist area(s) to ensure safe, effective, and patient-centred systems are in place.
In This Key Role, You Will
- Deliver patient-centred specialist pharmaceutical care to patients in specified clinical area(s).
- Communicate effectively, placing the person at the centre of any interaction.
- Deliver patient education to ensure the safe and effective use of medications and devices.
- Collaborate with the wider pharmacy and multidisciplinary team; actively participate in consultant ward rounds and multidisciplinary team meetings.
What You’ll Bring
- Masters of Pharmacy Degree
- Registered member of General Pharmaceutical Council
- Independent prescribing qualification or willingness to work towards it
- Evidence of postgraduate training and professional development in clinical pharmacy practice (e.g., Post Registration Foundation Framework / 1st-year clinical diploma)
- Relevant post-registration experience in clinical pharmacy
- Ability to effectively monitor patients’ therapeutic regimens to ensure effectiveness and reduce toxicity.
- Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment.
