CLASSIFICATION TITLE:

CLASSIFICATION MINIMUM REQUIREMENTS:
Master’s degree in an appropriate area; or a bachelor’s degree in an appropriate area and two years of relevant experience.

JOB DESCRIPTION:
The Clinical Case Manager functions as a member of a multidisciplinary team (headquartered in Lecanto, Florida) organized to provide and coordinate services for victims of child abuse and neglect and their families. This position will travel to outlying counties to provide CPT services in rural communities.
- Interviews children, family members, and significant others (as needed) to obtain medical and psychosocial information. Follows child through the process of intake, interview, medical exams, and follow up. Present with medical personnel for clinic and hospital visits. Arranges and conducts team staffing and participates in the development of recommendations. Hosts, organizes, leads,



and summarize staffings. Coordinates client services and referrals to community agencies.
- Completes assessments and sends reports to the protective investigators within the timeframes consistent with the program requirements and best practice. Maintains client records consistent with program requirements. All division performance measures set by the Assistant Director and must be met.
- Participates in “on-call” rotation including nights, weekends and holidays. Provide direct services and completes intakes and case notes during on-call. All designated paperwork and other requirements are completed during the on-call shift.
- Travels to 12-county catchment area to attend court proceedings, conduct trainings for community partners, and provide CPT services as assigned by Team Coordinator.

EXPECTED SALARY:
$50,000-$53,000 annually
