Full-Time Registered Nurse – Home Hospice
We are hiring a Full-Time Registered Nurse for a Hospice Case Manager role in the home health setting, caring for adult and geriatric patients in their homes. This position offers a adaptable, self-directed schedule — you coordinate directly with your patients, allowing you to structure your day around your life. Home Health or Hospice experience is not required — we welcome RNs with at least 1 year of experience with adult or geriatric medical/surgical patients in a hospital, SNF, or similar setting. What You’ll Do:
- Provide in-home hospice nursing care for adult and geriatric patients
- Coordinate and manage each patient’s care plan
- Maintain timely, accurate documentation (support and training provided)
- Travel within your assigned area for patient visits
- Collaborate with the hospice care team and family members

Requirements:
- Active RN license in the state
- Must be a U.S. Citizen or Green Card holder
- At least 1 year of experience with adult/geriatric med-surg patients
