The Role
We are seeking a motivated and forward-thinking Occupational Therapist to join our Medicine of the Older Adult service. This is an excellent opportunity to contribute to the delivery of person-centred, high-quality rehabilitation for older adults within a well-established multidisciplinary team. The successful candidate will manage a diverse caseload of patients presenting with frailty, deconditioning,
delirium and complex multi-morbidity.
You will undertake comprehensive occupational therapy/ Physiotherapy assessment and rehabilitation, delivering targeted, goal-oriented interventions to maximise independence and functional recovery. You will play a key role in supporting safe, effective and timely discharge in line with Discharge without Delay principles, working closely with patients, families and partners across health and social care. The role requires strong clinical reasoning, risk assessment and decision-making skills within a fast-paced environment.
