Job Description
- Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. Recognizes the patients, their family/designated care partner(s) in providing safe, compassionate and coordinated care based on respect for patient's personal preferences, priorities, needs,



and cultural beliefs and values.
- Communicates with and supports the interprofessional teams and services, patient, family and designated care partners. Serves as a clinical resource regarding policies and procedures, patient standards, performance improvement, and risk management issues.
- Collaborates with the medical staff to facilitate consistency of interdisciplinary plans of care to ensure quality patient care. Assesses patient needs and makes care recommendations based on the patient’s physical, psychosocial and cultural needs, age and functional status. Facilitates timely patient throughput optimization for all phases of care including admissions, transfers, and discharge optimization of patients on the designated unit. May participate in plan of care rounds depending on service and unit protocols.
